Coaching Informed Consent Agreement
I, Karen Yeh, JD, LMFT, have a Master of Arts in Counseling Psychology as well as a Master of Science in Electrical Engineering and Computer Science and a Juris Doctorate. I follow the ethics of the International Association of Coaching. I am also a licensed Marriage and Family Therapist in the state of Californian and have practiced as a therapist for ten years. Licensed Marriage and Family Therapists are licensed and regulated in the state of California by the Board of Behavioral Sciences.
Coaching
Coaching is not intended to diagnose, treat, or prevent a mental-health disorder or to replace psychotherapy, psychiatric care, medical care, or other appropriate professional treatment. In certain cases, I may require that a client be in treatment with a mental health professional.
While I am both a coach and a licensed mental-health professional, when providing services under this agreement, I am acting in the role of coach.
My mental-health license, clinical training, and professional experience may inform my awareness of client safety and appropriate referrals; however, the existence of that license does not mean that every coaching interaction constitutes psychotherapy or mental-health treatment. The client has the right to ask me at any time to clarify which professional role I am acting in.
Coaching may be paused, modified, terminated, or replaced with an appropriate referral if I determine that:
the client needs exceed the scope of coaching;
significant or untreated mental-health symptoms are interfering with coaching;
the client requires clinical assessment or treatment;
there is an immediate or significant safety concern;
another professional would be better suited to address the client’s needs; or
continuing the coaching relationship could create an ethical, professional, or boundary concern.
In certain cases, I may require that a client be in treatment with a mental health professional and/or may require collaboration with the therapist. If the client does not agree, I may terminate the coaching relationship and immediately refund any remaining balance. The client may return when they are in treatment.
Confidentiality and Privacy
I will respect the privacy of information shared during coaching. However, because coaching is not necessarily being provided as psychotherapy, the confidentiality protections applicable to coaching may differ from those applicable to a formal therapeutic relationship. Confidentiality may also have legal or ethical limitations, including circumstances involving safety concerns, suspected abuse or neglect where reporting is legally required, court orders, or other circumstances required or permitted by applicable law.
I may maintain records relating to coaching, including scheduling, payment, agreements, goals, session notes, communications, or other information reasonably necessary for providing and administering coaching services.
Emergencies and Crisis
Coaching is not an emergency or crisis service.
I do not provide continuous monitoring or 24-hour crisis support through coaching.
If you are experiencing an emergency, are at immediate risk of harming yourself or someone else, or require immediate medical or psychiatric attention, contact 911, go to the nearest emergency department, or contact an appropriate crisis service.

Fees and Policies
Clients may purchase packages of 6 50-minute sessions for $3,600. There is also a one-time fee of $300 for an initial intake session. There is cancellation fee of $300 if the cancellation is made within 48 hours of the scheduled session (though this will be waived one time if the session is rescheduled within the next 7 days).
Sessions will be booked on an as needed basis. There will be no standing recurring sessions. Packages will expire after 6 months. Client may repurchase packages at any time.
Client Acknowledgement and Consent
By signing below, I acknowledge that:
I have been informed that the services I am receiving under this agreement are coaching services.
I understand that coaching is distinct from psychotherapy and is not a substitute for mental-health treatment.
I understand that the provider is also a licensed mental-health professional and that the provider is acting in the coaching role under this agreement.
I understand that coaching may be paused, terminated, or transitioned to another provider if my needs exceed the appropriate scope of coaching.
I have had an opportunity to ask questions about coaching, confidentiality, privacy, fees, professional boundaries, and the distinction between coaching and therapy.
I understand that participation is voluntary and that I may withdraw from coaching.
I understand the limitations of confidentiality and privacy applicable to coaching.
I understand that coaching is not an emergency or crisis service.
I have received sufficient information to make an informed decision about participating in coaching.
